Rusted Perandus Scarab can be acquired by placing Janus Perandus in Intervention section of Immortal Syndicate and he is rank 1. After killing all the bosses (including Janus) in the Intervention Safehouse encounter, the scarab is stored in a chest in one of the rooms.
The scarab can also be acquired in Blight encounter.
