Ancient Orb is a currency item that can be used to reforge a unique item as another of the same item class.
Mechanics
- Ancient Orbs can only roll unique items into a different unique item in the core drop pool (e.g. not drop-restricted).
- The resulting item will always be different than the original item.
- The rerolled item will always be of the same item class (e.g. one-handed sword), and occupy the same number of inventory space in terms of size (e.g. a 3x2

- Ancient Orbs do not change the inventory size of an item, so 2x4-sized "tower shields" for example stay within that group.[1]
- Sockets and links are reforged when the item is applied. Influences and enchantments are removed when rerolling.
- Unique Jewels, Flasks or Maps cannot be re-rolled, although it was possible to do so prior to version 3.20.0.
- Unique flasks did not discern between life, mana, hybrid or utility flasks.
- Unique cluster jewels and Abyss jewels are considered in the same group as other regular jewels, though they are no longer Ancient Orb-able.[2]

Shift click to unstack., which tends to generally ignore item level of the base item when rolling into a unique item, Ancient Orbs appear to be influenced somewhat by the base item's item level in comparison to the potential outcomes' level drop level and/or required level for equipping. The exact correlation is unclear and appears to have changed, but for example:

- Interestingly, Titucius' Span has a level requirement of Level 30, the same as its basetype, which is too high for the ilvl.
The mechanics related to ilvl appear to have changed significantly over time, and many resources relating to it no longer apply. Additionally, more desirable uniques currently tend to have a higher drop level and required level, so it is generally recommended to use a higher ilvl unique for purposes of Ancient Orbing unless a player specifically wants a specific low-level unique item.
