It always has 3 enchantments when dropped. One determines the number of passive skills in that passive cluster, one determines the mod pool of the jewel, as well as adding effects to the small passives in the cluster. The last enchantment is fixed - 2 Added Passive Skills are Jewel Sockets.
Note that the unique version of this jewel does not have any enchantments.

All sources of delirium rewards have a chance to drop cluster jewels, such as a delirium chest inside a heist.
Selling any existing Large Cluster Jewels that without the jewel socket enchantment to a vendor, will remove any existing Jewel socket explicit modifier (of Potentialof Potential Intrinsic 1 Added Passive Skill is a Jewel Socket or of Possibilityof Possibility Intrinsic 2 Added Passive Skills are Jewel Sockets) and add the jewel socket enchantment. This does not include Unique Cluster Jewels.
Medium and Large Cluster Jewels will now drop with an additional enchantment effect that grants them 1 and 2 Jewel sockets respectively. These replace the modifiers that previously granted additional Jewel sockets, which can no longer appear on Cluster Jewels.
Selling any existing Medium and Large Cluster Jewels without this new enchantment to a vendor will remove any existing Jewel socket modifiers and add the new socket enchantment. This does not include Unique Cluster Jewels.
