Boneshatter can be used with a Mace, Sceptre, Axe or Staff.
Hitting an enemy with Boneshatter will grant you a Trauma stack, giving you more damage (2% to 4%) with the skill. However, you will also take Physical Damage per Trauma stack upon gaining a new one. Only one trauma can be obtained per hit, regardless of the number of enemies hit.

With a level 20 gem and N current Trauma stacks, you will take 194*(N+1) physical damage (before damage mitigation) upon your next attack.
Any Damage conversions to outgoing damage do not affect boneshatter's self damage, which is always taken as physical. However, damage taken as modifiers such as #% of Physical Damage from Hits taken as Fire Damage will convert the taken damage, allowing mitigation via elemental resistances. Trauma has no crit roll, and can only crit you if hits you take always crit you.[1][2]
