The Matriarch can be found in Oriath Square in Act 5.
Make sure you kill her before progressing further as changes in the area layout will make it impossible to return in case your are playing solo self-found. Joining another players instance of Oriath Square to return is possible though.
If you didn't kill monster and now you have The Ruined Square, you can still kill it without creation of new character. For this, you need someone who has Oriath Square. Then invite him in party and that person will open portal for you (use Portal Scroll). After that you can reach Oriath Square from Act 5 by using portal in city.
