The Matriarch is a unique Flame Mother in Oriath Square in Act 5, on the left side of the square. Presumably she is part of the Order of the Templar as a commander of the soldiers in that area.

She returns as The Risen Matriarch in The Ruined Square in Act 5 and The Rotten Matriarch in The Ravaged Square in Act 10.

League challenge warning

After defeating High Templar Avarius, the Act 5 layout will change and Oriath Square will no longer be accessible. Make sure you kill her before progressing further as changes in the area layout will make it impossible to return if you are playing solo self-found. It is still possible to return by using a party member's portal to join their instance of Oriath Square.
