Perforate is a meleeattackskill. The user slams the ground to cause multiple spikes to erupt from the ground, dealing area of effect damage. This skill interacts with Blood and Sand Stance, launching multiple spikes in a small area in Blood Stance (with less damage per spike) and launching the spikes outwards in a asterisk/star pattern in Sand Stance.
Increases to AOE cause the spikes in blood stance to be larger and more spread out, and the spikes in sand stance to be wider and travel further. While this improves clearing speed, this can decrease single target damage, especially against smaller foes.
In blood stance, each spike created can hit the enemy, resulting in several hits per skill use. This can be advantageous when taking advantage of impale, life gain on hit, instant leech, and other on-hit effects.
If used without an equipped/activated stance gem, perforate will default to the blood stance version.
The "75% less Damage while in Blood Stance" line applies to ailments inflicted by the spikes as well as hits.
Each spike in blood stance will do the full damage of the skill, reduced by 75%. This means the total, full listed damage of the gem is dealt if at least 4 spikes hit, with extra spikes resulting in even more damage. This makes blood stance perforate among the highest damage slams in the game, given ideal conditions (a large enemy, a high quality gem, and low AOE investment).
Sand stance spikes each do full damage, but only one spike per slam can hit an enemy, even if the slam is centered on the enemy.

Note that helmet enchantments are no longer obtainable since version 3.23.0 and can only be obtained in permanent leagues as already enchanted items from before this patch.
No longer has Added Attack Physical Damage at all gem levels.
It now has 218% Effectiveness of Base and Added Damage at gem level 1 (previously 155%), scaling up to 622% at gem level 20 (previously 265%).
