The melee swings differ in their damage type with the right-hand weapon applying corrupted blood to the player and the left-hand weapon damaging with 50% Physical & 50% Chaos as its source.

There is also an eight fireball variation with it being in a wider frontal cone throwing two either side and a larger fireball in the middle, this larger fireball has a 100% chance to ignite.
* Demon Orb Detonation: Kosis drops onto a Death Zone near a target, causing up to 8 consecutive ring-shaped shockwaves to radiate outwards, based on the size of the Death Zone. Ignores walls and obstacles. The shockwaves can also cause other Death Zones to detonate and create shockwaves on contact.
 
* Teleport Attack: Kosis teleports to a distant target, creating a small Death Zone at the previous location.
Kosis can instantly teleport to the player if they get too far away and will drop a smaller dome at his previous location, this can occur multiple times.
 
Kosis can use a lightning bolt attack that travels in a straight line from its casting point. The damage source of this attack is 100% lightning and has a 50% chance to shock.
 
Successfully breaking Kosis' energy shield while the dome is being channelled will interrupt the beam attack and [[stun]] the boss for a couple of seconds.
 
The fight is augmented by how many layers of delirium are present when encountered on a map and might explain any perception in low or reduced damage against the boss players feel as shown in the [[Delirium|scaling]] breakdown.
