A mysterious figure appears out of nowhere, trying to escape from something you can't see. She hands you a rusty-looking device called the Blood Crucible and urges you to implant it into your body.

The Blood Crucible fills with the blood of monsters you slay. Once it reaches a certain threshold, you're able to activate the device. This will instantly thrust you into a parallel reality, an alternate Wraeclast overrun by demons known as The Scourge. The more blood you have collected, the longer you can stay shifted into the parallel Wraeclast. But the longer you stay, the more damage you'll take.

Scourge league, also known as Path of Exile: Scourge, is a challenge league. It was launched on October 22, 2021 (UTC), alongside the Version 3.16.0 patch.

In this league, you are given a Blood Crucible to slot into your character. With it, you can charge up the device by slaying monsters in the area. Once it's sufficiently charged, you can activate to instantly shift into an alternate realm, overrun by monsters of the Scourge. The more it's charged, the longer you can stay in the realm, but the longer you stay, the more dangerous it becomes.

You can also hold items in the Blood Crucible to gather up corruption from Scourge monsters. Once you reach the required number of kills, you can transform the item and corrupt it, giving it one positive and one negative Scourge implicit modifiers. You can transform an item up to three times, with each attempt granting new possible corruption outcomes. Killing Scourge monsters also grants experience points towards the Crucible skill tree, which can enhance Scourge corruption outcomes.

Scourge monsters also drop corrupted currency items, which can only used on corrupted items to modify them.

3.16.0b	Upon encountering a Boss in Nightmare, the drain of blood from the Crucible is now paused for 5 seconds. The damage of Ravenous Bloodshaper's Blood Orb and Spike Barrage skills has been decreased. The damage of Demon Herder's Vomit and Beam skill has been decreased. Reduced the number of Monsters with Proximity Shield that can spawn in some Scourge encounters. The Corruption Absorbed progress bar now displays underneath items stored in the Blood Crucible, without you having to hover over them.
3.16.0	Introduced to the game. Fixed a bug in Scourge League where the Blood Crucible could mistakenly upgrade an explicit mod on unique items. We also plan to fix existing items that were affected by this.
