Sanity's Requiem is a quest given to the player by Zana in the storyline's epilogue.
Walkthrough
After defeating the four Guardians of the Void, enter the Shaper's Realm and defeat The Shaper.
The Shaper will drop The Shaper's Key on death. Use it to unlock the locked room in the Templar Laboratory near the Map Device. Speak to Zana to claim the 
Elder's OrbElder's OrbUpgrades any map on the Atlas to Tier 16Right click this item then left click on a map on the Atlas to upgrade it to tier 16.
