Mechanics
When a monster is killed in a Beyond-enabled instance, there is a 15% chance for a small red orb to appear on the ground. If four of these appear close to each other (roughly one screen), portals will open, spawning Beyond demons. Once a substantial number of Beyond demons have spawned in the instance, each pack spawned grants a stacking chance for the next pack in that area to be replaced by a boss. Only one Beyond boss can spawn per instance. Once a boss has spawned, Beyond portals will no longer spawn in that instance.
Demons are divided into three factions, and only one faction will spawn in any single instance. These factions are the Pale Scourge, the Flesh Scourge, and the Demonic Scourge.
Beyond is not a core game mechanic and cannot naturally spawn; however, they can spawn in maps altered by Kirac's map device mod, as well as from the passive skill of Atlas Passive Skill Tree. Certain nodes in Delve (Azurite Mine) also contain Beyond encounters. The legacy map mod OtherworldlyOtherworldly

Lore
Beyond Demons are interdimensional demons from parallel realities that attempt to enter the player's world. The Nightmare realm of the original Beyond demons has been usurped and replace by the Scourge.
The Order of the Templar were aware of the existence of these demons, but kept information about them locked away in the Forbidden Sanctum and in Forbidden TomeForbidden TomeArea Level: (68-83)Mundus noster cecidit. Daemones ubique sunt. Librum

Lycia, a former Templar turned heretic, came into contact with and formed a pact with Beidat, gaining immortality in exchange for the loss of her humanity, shackling her to the Forbidden Sanctum. Beidat uses Lycia to attempt to create a gateway into Wraeclast.
Chaos and the High Priests of the Vaal also appear to be associated with the Scourge in some way, creating the Blood CrucibleBlood CrucibleThe gateway of flesh, horror unbound,

The Breaker of Oaths also is aware of the presence of the Scourge in the Viridian Wildwood, seemingly diverted there by the powers of the Draíocht, possibly to protect Wraeclast.
The Last to Die states the Scourge were able to break through into Wraeclast in the future of every timeline except one, where they were vanquished by High Templar Venarius.
