Heretical Flare is a debuff from the monster skill of the same name. It deals physical damage over time. This is NOT a bleeding ailment.
It is used by the boss Cato, Defiler of Light and its map variants Barthol, the Corruptor. In the first phase of the boss (as Cato, Scholar of Light or Barthol, the Pure), he uses 
Divine Flare instead.
It is also existed in the boss room of Avarius, Reassembled (The Desecrated Chambers) and his/its map variant Woad, Mockery of Man (

Heretical Flare is in fact coded as a minion.[1] The "monster" is not targetable and does not have a nameplate. It has a skill that deals the actual damage. For Avarius, Reassembled's case, it is known as InvisibleIonKitavaCannonWickerman internally.[2]
