You are taking Physical Damage over time from a Heretical Flare

Divine Flare is a debuff from the monster skill of the same name. It is used by the boss Cato, Defiler of Light and its map variants Barthol, the Corruptor. In the first phase of the boss (as Cato, Scholar of Light or Barthol, the Pure), he uses Divine Flare instead.

It is also used by another unique monster Avarius, Reassembled and his map variant Woad, Mockery of Man. (unknown internal skill id)
