It is dropped by Delirium with the mod GlaciatorGlaciator
Monster Affliction or GlaciatorGlaciator
Monster Affliction. The monster would use a skill that has an id AfflictionSplitColdSpearGroundToss.
Trivia
Frozen ground shared with the same status icon with Veritania, the Redeemer's environmental debuff Creeping Ice.
