An Exalted Orb is a currency item that can be used to enhance a piece of rare equipment with a new random affix.

Obtaining

Exalted Orbs are extremely rare currency items that can be dropped by slain monsters, chests, and destructible containers. They also drop from Arcanist's Strongboxes.

Unlike most other currency, these orbs can't be obtained through vendors, except for a very unprofitable vendor recipe where the player trades an Mirror of Kalandra for two Exalted Orbs, five Regal Orbs and one Divine Orb.

Spending

Exalted Orbs can be used to add an affix to a rare item. This can only be used on an item with less than six affixes, since rare items can have at most three prefixes and three suffixes.

Its main use is for trading, however. It is the "gold standard" in the player-driven economy and is used to purchase the more expensive and rare items.
