You are taking Lightning Damage from a Lightning Beam skill
Piety Beam is a debuff from Piety when she use a skill to disable Malachai, The Nightmare's invulnerable shield during Act 4. Although this skill is aimed at Malachai, players and allies crossing through the beam will take Lightning Damage over Time and will be Shocked.
The icon is the same as Degeneration Aura (lightning damage variant), Scientist's Beam and Lightning Beam.
