Primary usage should get title without modifier
Since I expect the primary usage of "Herald of Thunder" would be the skill gem, I recommend using the unmodified title for it. The unique monster, or whatever the other usage is, should get the parenthetical modifier in the title. A disambiguation note can be added to the skill gem page to link users to where they need to go if they meant the other usage.
