You are taking Cold Damage over time for each manifestation of fear near you
Fearful is a debuff that deals cold damage over time. It is caused by the "manifestation of fear" near the player character. The "manifestation of fear" is water bubble like non-targetable object that can be found in several boss arenas.

Trivia
The debuff shared the same status icon graphic with chill ailment, chilled ground, Snow Cloak buff and "Cold Snap" debuff from Delve monster Unstable Weta. However, since Fearful is not an ailment, it cannot be avoided by the passive skills that grant the player character to avoid elemental ailment.
